Family Practice Physician, MD/DO

LCSW, Individual and Family Counseling – Full-time and Part-time Positions

Hill Country Health & Wellness Center in Round Mountain, CA is seeking candidates for an MD or DO with family practice or internal medicine background. Ideal candidate delivers care with an understanding of the social determinants of health; we will consider part-time or full-time opportunities. We are a rural Federally Qualified Health Center (FQHC), with a major LEED-certified building expansion project recently completed, and have loan repayment programs available. We have a warm and caring staff and our goal is to help people make choices every day that help them be healthier and happier. Integrated medical, dental, mental health and wellness services are all offered on site.

LCSW – Individual and Family Counseling

Candidate will provide individual, group and family counseling to clients of all ages. Behavioral health services are integrated into primary medical care, with a shared chart and treatment approach. Depression, anxiety and PTSD are frequent issues. Flexibility and collaboration are valued.

Ideal candidate is easy-going, flexible, likes collaboration with colleagues, and values the philosophy of holistic and integrative care. Candidates must be experienced with children and families. Integrated medical, dental, mental health and wellness services are all offered on-site. Ideal candidate can work both independently and as part of a treatment team.

LCSW – Integrated Behavioral Health Consultant (BHC)

The BHC works as an integral member of the primary care team.  Medical patients may receive brief counseling as part of their medical visit. The focus is on behavior change to improve health. The BHC also refers patients for longer term counseling to the Behavioral Health Department. This is a fast paced position that requires good communication skills and a pro-active style. LCSW – School-Based

Come join our school-based counseling team. This LCSW will work with children, adolescents, and families in schools in eastern Shasta County, (Burney, Fall River Mills, and Montgomery Creek) 2-4 days per week during the school year.

The ideal candidate for this position has exceptional skills in mental health assessment, treatment planning, and intervention with children, adolescents and their families. This position involves working independently and as a team with other clinicians, teachers, school administrators, and other agencies/professionals at multiple locations. Flexibility, creativity, and strong self-initiative is essential.

Hill Country Health and Wellness Center in Round Mountain is seeking candidates to fill a full-time Medical Assistant position in its clinic in Round Mountain, CA. Must have reliable transportation. Schedule is four ten-hour days, Monday-Friday.

Duties include assisting providers in all aspects of the delivery of care, maintaining exam rooms, monitor and record patient vitals and symptoms, prepare patient for examination or procedure as directed by provider, perform routine procedures and tests as instructed, maintain patient information, and contact as instructed.

Hill Country Health and Wellness Center in Round Mountain is seeking candidates to fill a full-time Referral Coordinator position in its rural community health clinic. Medical experience preferred.

Duties include faxing and following up on patient referrals, calling patients, handling prior authorizations and 340B prescriptions.

We are looking for someone who has excellent attention to detail, excellent communication and customer service skills, is a quick learner, has a great work ethic, is able to keep up in a fast paced work environment, and is able to contribute and work in a team atmosphere. Familiarity with medical terminology is strongly recommended.

Full Time : $14/hr

Hill Country Health & Wellness Center is looking for a Behavioral Health Care Coordinator to join our team. Position is located at our new facility in downtown Redding.

Ideal candidate is easy going, flexible, likes collaboration with colleagues, and values the philosophy of holistic and integrative care. Job duties include coordination of psychiatric services — scheduling patients, Rx’s that are sent to local pharmacies. Self-management goals and the SHA assessments with new Partnership patients. Receive warm hand-offs from medical providers to assist patients with basic needs and health coaching. Outreach — going to meet clients out in the community, transporting clients to necessary appointments. Meetings — i.e. Rural Mental Health, Multi-Disciplinary Team. Training — important to stay current on MHSA laws, 5150 rules and job improvement trainings. FSP — helping the FSP case manager when appropriate with managing caseload.
Behavioral Health experience desired.

We are seeking a reliable Security Guard to work swing shift who can handle interior and exterior security through manual observation at our new Care Center facility located in Redding. We offer free mental health services to our community 365 days a year and works with clients that need our support. .

This candidate will be responsible for monitoring the interior and exterior of the facility, checking to make sure that certain doors are locked, reporting any suspicious activity, contacting local law enforcement if needed and assists with deescalating situations if they arise.

The ideal candidate for this position has a background in security, EMT and a professional disposition.
Must be able:
● Maintain a clam demeanor
● Have presence and the ability to deescalate crisis situations.
● Prior training in emergency medical
● Must be willing to be CPR/First aid trained.
● Must be will take online security training.
● Must be easy going, flexible, and has a kind heart for our community clients.
● Able to work 2pm-11pm with a strong possibility of working one Saturday or Sunday. Our Saturday and Sunday shifts start at 11am and end at 11pm.

Hill Country Health and Wellness Center is seeking a highly competent and genuinely compassionate Case Manager to join our newly forming Assisted Outpatient Treatment (AOT) Team (1 clinician and 2 case managers). This position is based out of Hill Country’s CARE Center in Redding and offers a rare opportunity to have a profound impact, both through intensive work with individuals with complex needs and through developing systems of care for some of our most vulnerable community members.

AOT is a unique evidence-based care approach for persons with a serious mental illness who are repeatedly at risk for psychiatric hospitalization, homelessness, or arrest, and who have been reluctant to participate in ongoing mental health treatment. This model has been very successful in other Counties around California and offers an effective and compassionate option for promoting the safety, stability and well-being of those who participate. An essential aspect of Hill Country’s AOT program is a “whatever it takes” philosophy that empowers and supports participants in creating their own plans and goals for short term recovery and long term wellness, with AOT staff providing very high levels of support and resources to help clients achieve their goals. A small caseload, capped at 20 individuals, is shared by a clinician and two case managers who work together to engage potential clients, complete a needs assessment, develop and implement care plans, and access appropriate community resources. Crisis prevention, intervention, stabilization, and recovery are achieved through team-based care strategies that are proactive, adaptive and personalized. The AOT team works collaboratively with Hill Country’s new urgent mental health care teams at the CARE Center as well as the staff at our integrated health care clinics in Redding and Round Mountain. These sites provide outpatient primary health care, mental health services, addiction treatment, medication support, and several programs serving people with complex health issues.

In addition to having exceptional case management skills, the ideal candidate’s professional experience will reflect warmth, ingenuity, integrity, resilience, flexibility and, most of all, deep satisfaction working with people living with a serious mental illness. A sense of humor is a must as are strong ethical reasoning and excellent oral and written communication skills. This is a job for someone who is highly motivated to improve outcomes for people with serious mental illness through direct service and effective advocacy with family members and service providers. Hill Country has a reputation for being friendly and trustworthy collaborators with other agencies and the Case Manager must be a natural fit.

Hill Country Health and Wellness Center in Round Mountain is seeking candidates to fill a full-time Front Office Float position in its Round Mountain medical clinic.

This person will be trained in all our departments – medical/counseling/dental, and be able to fill in wherever needed, including at our Redding clinic locations. This person will love working with patients and help provide excellent service to our community. Ideal candidate will have health clinic front office experience, including patient intake, appointment scheduling, chart preparation, insurance coordination, electronic practice management software experience, and have working knowledge of MS Office suite, Excel, Word, Outlook.
