Family Practice Physician, MD/DO

LCSW, Individual and Family Counseling – Full-time and Part-time Positions

Hill Country Health & Wellness Center in Round Mountain, CA is seeking candidates for an MD or DO with family practice or internal medicine background. Ideal candidate delivers care with an understanding of the social determinants of health; we will consider part-time or full-time opportunities. We are a rural Federally Qualified Health Center (FQHC), with a major LEED-certified building expansion project recently completed, and have loan repayment programs available. We have a warm and caring staff and our goal is to help people make choices every day that help them be healthier and happier. Integrated medical, dental, mental health and wellness services are all offered on site.

LCSW – Individual and Family Counseling

Candidate will provide individual, group and family counseling to clients of all ages. Behavioral health services are integrated into primary medical care, with a shared chart and treatment approach. Depression, anxiety and PTSD are frequent issues. Flexibility and collaboration are valued.

Ideal candidate is easy-going, flexible, likes collaboration with colleagues, and values the philosophy of holistic and integrative care. Candidates must be experienced with children and families. Integrated medical, dental, mental health and wellness services are all offered on-site. Ideal candidate can work both independently and as part of a treatment team.

LCSW – Integrated Behavioral Health Consultant (BHC)

The BHC works as an integral member of the primary care team.  Medical patients may receive brief counseling as part of their medical visit. The focus is on behavior change to improve health. The BHC also refers patients for longer term counseling to the Behavioral Health Department. This is a fast paced position that requires good communication skills and a pro-active style. LCSW – School-Based

Come join our school-based counseling team. This LCSW will work with children, adolescents, and families in schools in eastern Shasta County, (Burney, Fall River Mills, and Montgomery Creek) 2-4 days per week during the school year.

The ideal candidate for this position has exceptional skills in mental health assessment, treatment planning, and intervention with children, adolescents and their families. This position involves working independently and as a team with other clinicians, teachers, school administrators, and other agencies/professionals at multiple locations. Flexibility, creativity, and strong self-initiative is essential.

Hill Country Health and Wellness Center is seeking a highly competent and genuinely compassionate licensed mental health clinician (LCSW/LMFT/LP) to lead our newly forming Assisted Outpatient Treatment (AOT) Team (1 clinician and 2 case managers). This position is based out of Hill Country’s CARE Center in Redding and offers a rare opportunity to have a profound impact, both through intensive work with individuals with complex needs and through developing systems of care for some of our most vulnerable community members.

AOT is a unique evidence-based care approach for persons with a serious mental illness who are repeatedly at risk for psychiatric hospitalization, homelessness, or arrest, and who have been reluctant to participate in ongoing mental health treatment. This model has been very successful in other Counties around California and offers an effective and compassionate option for promoting the safety, stability and well-being of those who participate. An essential aspect of Hill Country’s AOT program is a “whatever it takes” philosophy that empowers and supports participants in creating their own plans and goals for short term recovery and long term wellness, with AOT staff providing very high levels of support and resources to help clients achieve their goals. A small caseload, capped at 20 individuals, is shared by the clinician and two case managers who work together to engage potential clients, complete a comprehensive assessment, develop and implement care plans, and access appropriate community resources. Crisis prevention, intervention, stabilization, and recovery are achieved through team-based care strategies that are proactive, adaptive and personalized. The AOT team works collaboratively with Hill Country’s new urgent mental health care teams at the CARE Center as well as the staff at our integrated health care clinics in Redding and Round Mountain. These sites provide outpatient primary health care, mental health services, addiction treatment, medication support, and several programs serving people with complex health issues.

In addition to having exceptional clinical skills, the ideal candidate’s professional experience will reflect warmth, ingenuity, integrity, leadership, resilience, flexibility and, most of all, deep satisfaction working with people living with a serious mental illness. A sense of humor is a must as are strong ethical reasoning and excellent oral and written communication skills. This is a job for someone who is highly motivated to improve outcomes for people with serious mental illness through direct service and effective advocacy with family members and service providers. This position will also involve working directly with leaders in public mental health, law enforcement, court systems, and other community agencies to establish the AOT system of care. Hill Country has a reputation for being friendly and trustworthy collaborators with such agencies and the AOT Team Leader must be a natural fit.

LMFT or LCSW – school based services
Hill Country Health & Wellness Center in Round Mountain is looking for a licensed clinical social worker to join our school-based counseling team. Hill Country Health and Wellness Center is creating a team of compassionate, creative, and highly-motivated clinicians and case managers/recovery coaches to wrap around the complex needs of people experiencing a non-life threatening mental health crisis.

Our team is based in downtown Redding and will work together to create a warm, safe, and welcoming environment for recovery. We are seeking highly competent clinicians who share our philosophy and vision towards integrated and whole-person care.

Medical Records Clerk

Hill Country Health and Wellness Center in Round Mountain is seeking candidates to fill a full-time Medical Records Clerk position in its rural community health clinic. This person will be trained to work in the medical records department.  Hours are Tuesday – Friday 4  – 10/hour shifts, 8 am to 6 pm

Ideal candidate will have health clinic experience and meet the following criteria

  • Conduct chart auditing which includes addressing what’s on the front of the chart.
  • Check to make sure prescriptions called or faxed in.
  • Make sure all data is completely entered such as date and provider signature.
  • Make sure all papers are filed in the proper places.
  • Make sure all faxes were done and check to see if paperwork needs to be completed and copied and mailed or faxed
  • May assist in data entry for medical
  • Must have working knowledge of MS Office suite, Excel, Word, Outlook.We are looking for someone with excellent attention to detail, strong all around administrative skills, excellent communication and customer service skills, very computer savvy, a quick learn, good work ethic, and able to keep up in a fast paced work environment. Must be able to contribute and work in a team atmosphere.

Registered Dental Assistant

Hill Country Health and Wellness Center in Round Mountain is seeking candidates to fill a full-time experienced Registered Dental Assistant position in its rural community health clinic, an experienced Dental Assistant would be considered. Round Mountain is about 30 miles NE of Redding on Hwy 299E; only those willing to make the drive daily should apply.

Duties include assisting in all aspects of the delivery of care, maintaining exam rooms, monitor and record patient information, prepare patient for examination or procedure as directed by provider, perform routine procedures, maintain patient information, and contact as instructed. Must have strong working knowledge and understanding of dental procedures, terminology and techniques. Must demonstrate ability to set up and prepare dental equipment and instrumentation, and perform x-rays, in accordance with established regulations and guidelines.

Candidates must have RDA; some work experience is preferred. We are looking for someone who has excellent attention to detail, excellent communication and customer service skills, is a quick learner, has a great work ethic, is able to keep up in a fast paced work environment, and is able to contribute and work in a team atmosphere.
