Hill Country Awarded $20,000 from Blue Shield Foundation
Hill Country Health and Wellness Center recently received a grant award for $20,000 from Blue Shield Foundation of California. The money will be used to help prepare for the implementation of electronic health records in mid-2012.
Positive Health Outcomes: Diabetes
In 2003, Shasta County’s death rates from diabetes were 30% higher than California’s state rate. In 2007, Hill Country participated in the HRSA Diabetes Collaborative; this comprehensive approach to disease management has improved our outcomes significantly.
In 2009 - 2010, Hill Country Health and Wellness Center provided Hemoglobin A1c blood tests for 95% of its diabetic patients, well above the statewide average of 75%. In addition, our programs are helping our patients to manage their diabetes: 79% of A1c-tested patients at Hill Country scored below 9, which is well above the statewide average of 58%. An A1c level above 9 indicates diabetes that has been uncontrolled for some time.
Hill Country provides a comprehensive obesity and diabetes program for our patients, which includes nutrition and cooking classes, as well as exercise programs. A .25 mile walking trail around the perimeter of the clinic is available to the public. If you or a loved one suffers from obesity and/or diabetes, visit our clinic to see how we can help you.
